About Us

Unite Colorado is a coordinated care network consisting of healthcare, government, nonprofit, and other organizations. These network partners use Unite Us’ software to securely identify, deliver, and pay for services that address the needs of individuals within their communities.

The network is supported by Unite Us team members focused on customer and community engagement, network health and optimization, and user support.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Food Bank of the Rockies believes all food-insecure individuals deserve access to nutritious and culturally appropriate food. In the last year, we’ve been able to serve over 700,000 individuals facing food insecurity with 54.7 million meals. We are excited to collaborate with Unite Colorado and other human service organizations to address root causes of food insecurity.

Summit County is excited to partner with Unite Us, to improve access and streamline referrals among Summit County service organizations. The COVID-19 pandemic has hit our community hard and laid bare the need for deeper collaboration among partners. Often times people have multiple needs including housing, food access, and mental health. Unite Us will remove barriers, so that people receive needed supports faster. With a seamless referral system, community members will not need to tell their story over and over again, and the Summit Community can provide the wrap-around support needed in order to help our community heal.
