Elevating social care to the
same level as health care.

Now more than ever, people are turning to their communities during times of need. When a person needs support, they may not always go to a hospital. Instead, the first line of care often lies within their schools, churches, libraries, and community groups.

  • Our cross-sector collaboration software transforms community health from the ground up.
  • CBOs can streamline the process of sending and receiving referrals in order to deliver more holistic, whole-person care and track the impact of their work.
  • We are dedicated to ensuring nonprofit leaders have a voice at the table, alongside our partners in government, healthcare, and other sectors.

What We Do

We have a proven track record of improving outcomes for communities, big and small, across the country. We enable our nonprofit and community-based partners to:

Join a network of like-minded, accountable organizations dedicated to supporting people’s health and well-being

Easily and efficiently connect clients to a variety of local resources and increase the visibility of your services

Track outcomes of services delivered to know if a client received the help they needed

Demonstrate your organization’s impact with data to support grants and other funding opportunities

Adopting new technology can be a challenging task, which is why Unite Us is committed to supporting partners long after they sign in for the first time. We provide ongoing user support through live and on-demand training, Help Center articles, and a Technical Support team that is available via email, chat, and phone Monday–Friday from 8:00 a.m.–11:00 p.m. EST.

Who We Are

What sets us apart is our cross-collaboration software, our intentional community engagement practices, and the local expertise our team brings to the table. Our customer and community success team members have an intimate, firsthand understanding of the nonprofits we serve. Many have experience in public health, nonprofit management, and community-based care coordination. Don’t be surprised if you cross paths with them volunteering at a community outreach event or serving as a member of the local Rotary Club; they are invested in their communities in more than one way.

“By utilizing the Unite Us network, our staff will be able to easily develop relationships with community partners, enabling us to improve the lives of individuals living in our communities. Our staff will be able to help residents get a range of services that will allow them to age-in-place with dignity, get back into the workforce, or find health care to maintain mental and physical stability.”

Brianna Fimbres
Community Services Manager for Cesar Chavez Foundation

“When you’re about to be evicted, when your kids are hungry and you don’t have food—that is a lot of stress, and it reduces the quality of health that individuals have. To be able to know, ‘I’ll be able to receive these services to help feed my kids’—that’s a lot of health benefits that we see right away.”

Joshua Chitalo
CEO of Mount Carmel HELPS, Inc.
