Social factors like access to healthy food and safe, affordable housing have a major impact on health. Even before the pandemic, many people’s basic needs were not being met, and COVID-19 has only made that worse, particularly in communities of color. It is exciting to be part of a robust, online platform where referrals and ongoing coordination enable health systems and safety-net resources to stay connected as we work together to take care to the next level for the people and families we serve.

We believe data is power in helping TACHC reach its mission of health equity, access, and quality for all Texans. Our partnership with Unite Us will give our community health centers the tools and data they need to empower decision-making to address non-medical drivers of health, which ultimately improves a patient’s health outcomes.

Unite Us allows us to focus on the patients’ medical needs while having access to community resources to meet the patients’ non-medical needs. By having access to resources through Unite Us, we are able to better help patients meet all their needs, which ultimately helps achieve better health care.
