IHN-CCO Health Related Social Needs Form
Health Related Social Needs (HRSN) benefits are for members with life changes listed by Oregon Health Plan.
The life changes covered for HRSN benefits are one of the following.
- A member with a history or current Child Welfare situation.
- Adults and youth leaving some facilities for mental health treatment.
- A member who is new to having both Medicare and Medicaid health plans.
- A member is who released from a detention center, jail, or prison within the past year.
- Members who are at risk of homelessness.
Climate Devices help keep members healthy.
The rules for covering climate devices include:
- An IHN-CCO plan that includes medical and behavioral health coverage.
- AND has one of the life changes listed above.
- The member cannot have already received the same or similar device in the last 3 years.
- The member can safely use the device in their home.
- The member lives in their own home/apartment, lives with friends/family or lives in a camper/trailer. (A camper/trailer will need a reliable power source, can be a generator).
- AND a health condition:
- For air conditioners, a health condition that gets worse during hot weather.
- For heaters, a health condition that gets worse when the member gets very cold.
- For air filters, a health condition that gets worse when air quality is poor.
- For mini fridges, a health condition that requires temperature-controlled medication.
- For portable power supply, a health condition that requires a powered medical device.
When you fill out this form our team will review your information to see if you could be covered for an HRSN Climate Device.
