Unite Us Celebrates Record Impact in 2024, Surpassing 2023 Impact Milestones by Nearly 60% in Three Quarters

Unite Us’ closed-loop referral platform unites healthcare, health plans, government, nonprofit, and private sector partners to address social drivers of health (SDoH) comprehensively and effectively.
In the first nine months of 2024, Unite Us connected millions of individuals with services, a notable increase of nearly 60% compared to its entire year in 2023. In total, Unite Us has facilitated more than 30 million screenings, referrals, and resource shares within its platform.

These collective efforts play a crucial role in addressing the health-related social needs (HRSNs) of individuals and families in need, demonstrating that when organizations come together, they can create lasting and impactful change in communities.
Key performance metrics for 2024 from across Unite Us’ nationwide network as of the end of September include:
- A nearly 60% increase in the number of individuals served compared to 2023
- A 74% increase in funds distributed, amounting to $76 million distributed through Q3
- A 39% increase in service utilization across the top three services, Food insecurity, Housing and Shelter, and Individual and Family Support in 2024

This includes being the country’s foremost leader in supporting states in the 1115 Medicaid waiver demonstration, which expands the use of Medicaid funds for social services.
Notable achievements include:
- In New York’s just-launched 1115 Medicaid Waiver Demonstration, nine out of 13 Social Care Networks selected by the State chose Unite Us as their platform to serve nearly 75% of Medicaid members. Unite Us technology will integrate Medicaid member eligibility verification, offer scalable payment solutions, and deliver reimbursements for community-based organizations (CBOs), enhancing their capacity to serve.
- In North Carolina the company continues to see remarkable results administering its 1115 Waiver Expansion with the North Carolina Healthy Opportunities Pilot through its statewide coordinated care network built on Unite Us technology. As reported in HOP’s April report, the state delivered nearly half a million services, saved the state an average of $85 per member per month, and reimbursed more than $107 million to CBOs across the state, resulting in the delivery of over 560,000 services, as of August 31, 2024.
- In Oregon, Coordinated Care Organizations using Unite Us Payments have kicked off their work to reimburse community-based organizations for devices distributed under the Health-Related Social Needs (HRSN) Climate benefit with 95% of invoices currently accepted, paid, or in progress. The state’s use of this functionality will expand to support the Housing and Nutrition benefits due to launch in November 2024 and January 2025.

- Live Chat: A secure chat feature that helps facilitate the real-time collaboration required for whole-person care.With Live Chat, customers can securely chat with other users at partner organizations based on shared referrals, cases, and clients that are directly managed between the two organizations; reference previous conversations for quick and efficient follow-up; and answer questions from other organizations directly involved in a client’s care in real-time.
- Unite Us Closed-Loop Referral System: The Unite Us Platform is continually updated to exceed industry standards and allow organizations to focus on addressing social care needs, not backend administrative work. Recent updates focused on expanding interoperability with leading healthcare Electronic Health Records (EHRs) and Customer Relationship Management applications—to ensure clients do not need to log into a different system—streamlining built-in screening tools and enhancing dashboards that show metrics to help clients quantify the impact of their work.
