About Us

Unite Rhode Island is a coordinated care network consisting of healthcare, government, nonprofit, and other organizations. These network partners use Unite Us’ software to securely identify, deliver, and pay for services that address the needs of individuals within their communities.

The network is supported by Unite Us team members focused on customer and community engagement, network health and optimization, and user support.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Unite Rhode Island is sponsored by Rhode Island Office of Veteran ServicesLifespan, the Rhode Island Executive Office of Health and Human Services, Blue Cross Blue Shield of Rhode Island, and Rhode Island Department of Education.

The Rhode Island Department of Veteran Services also oversees the RIServes, a Unite Us-powered network that focuses on service members, veterans, and their families.

The Unite Rhode Island implementation team includes the United Way of Rhode Island which acts as the statewide coordination center for any non-veteran Rhode Islanders.

The newly created partnership between Kids’ Link and Unite Us will streamline the coordinated care network of health and social service providers in our state. As part of the Central Providence Health Equity Zone Initiative, ONE Neighborhood Builders runs a cohort of 12 Community Health Workers. Having our CHWs use the Unite Us platform has allowed us to keep track of their efforts in nearly real time, identify areas that need extra support, and quantify all of their important work for our funders and partners. This platform affords us the evidence to back-up what we already know – that Community Health Workers are invaluable for improving the health of their neighborhoods.
