Built in partnership with Contexture, the Arizona Health Care Cost Containment System (AHCCCS), and other local partners, the CommunityCares network, powered by Unite Us, is transforming health outcomes across the state of Arizona.
Built on Unite Us infrastructure, CommunityCares is a single, statewide referral network that addresses non-medical needs in Arizona. This collaborative care network brings together healthcare providers, government agencies, payers, and community-based organizations (CBOs) to securely identify, deliver, and pay for services that meet the needs of individuals in their communities.
Joining the network is free of charge for nonprofit CBOs providing social services—and it’s growing larger every day. Together, we’re creating lasting positive change and unlocking the potential of every community.
