Unite Us is excited to announce that we are currently requesting proposals from community-based organizations (CBOs) to act as community conveners in building out and growing coordinated care networks of health and social care providers nationally. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive secure, electronic referrals.

This opportunity requires selected partners to act as local champions in the implementation of the network. Responsibilities of the convener include maintaining active users of the Unite Us Platform and facilitating key network growth milestones, such as successfully onboarding CBOs and co-hosting community events. Awardees are eligible to receive up to $50,000 per contract term.

Once fully implemented, this multi-stakeholder collaboration will ultimately bring together CBOs, healthcare partners, government entities, and local champions to utilize our platform to create an accountable network that addresses social needs and improves health across communities.
