How North Carolina’s Healthy Opportunities Pilot (HOP)
is Changing Lives
Launched by North Carolina Department of Health and Human Services (NCDHHS) in 2022, the pilot program invested $152M providing services that address Social Determinants of Health (SDOH) for Medicaid members. The program focuses on improving health outcomes by funding non-medical services that impact well-being, such as housing, food security, transportation, and interpersonal safety.

Unite Us powers the technology infrastructure behind the NCDHHS’ groundbreaking 1115 Waiver program, the Healthy Opportunities Pilot (HOP).
NCDHHS chose Unite Us to streamline critical processes, including human services referrals, service authorizations, program enrollment, and reimbursement for services to address essential health-related resource needs like food, housing, transportation, and support for interpersonal violence and toxic stress. This is all made possible through NCCARE360, North Carolina’s statewide care network.
