Collaborating for a healthier Arizona.

As everyone who has worked in or navigated social services knows, it can be overwhelming for folks trying to access the different programs and assistance they need. We’re enthusiastically joining CommunityCares to co-create a community of care where no one is left to navigate these systems on their own. And ultimately, we see a future where Arizona is addressing all systemic issues at their root causes, strengthened by this community to propel that future forward.

Collaboration is key to Zion Institute’s strategy for growth and sustainability for our community. Becoming a Unite Us partner is going to increase our collective impact exponentially and help Zion Institute diminish the rate of poverty in South Phoenix.

I am so excited lately because I found out that Unite Us is now here in Arizona. This fantastic technology company is identifying community partners to enhance our ability to communicate, share resources and change the perception of our disadvantaged communities to ADVANTAGED!

One of the efforts we feel is foundation to our work around whole person care is the establishment of CommunityCares, Arizona’s SDOH referral system. As Arizona’s trusted HIE, Contexture is the ideal community partner to implement this necessary SDOH technology. And by Contexture, Solari, and 2-1-1 Arizona joining forces with Unite Us, we see CommunityCares as a real opportunity to connect community resources with individuals’ social needs, ultimately resulting in improved member health and wellness.
