Building connections for a healthier Colorado.
Unite Colorado is a coordinated care network of health and social service providers to address people’s social needs, and improve health across communities. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs and improve health across communities.
The network is supported by a Colorado-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

What People Are Saying
At The Center for African American Health, we see the health disparities that our community members face and we are striving to offer more and make connections to services that reflect the needs of community members of color. Professionally and personally, I am really excited to connect our most vulnerable community members with the care they need, increase accessibility and track outcomes. I believe that engaging in Unite Us is going to bring us together and streamline resources.
Food Bank of the Rockies believes all food-insecure individuals deserve access to nutritious and culturally appropriate food. In the last year, we’ve been able to serve over 700,000 individuals facing food insecurity with 54.7 million meals. We are excited to collaborate with Unite Colorado and other human service organizations to address root causes of food insecurity.
Having worked in healthcare and grassroots non-profit, this is the sort of solution we’ve been looking for collectively to support people seeking assistance in a more robust way. Having gone through the training and webinars, I’m confident that this Unite Us platform is both inclusive for the big and small players, as well as user-friendly.
Helping people get well from addiction is the most important work we do. We were eager to join the Unite Us network to extend our services across the region. We often make referrals to outside entities – this is an important step to ensuring we’re meeting the needs of our members.
We are most excited to be part of the Colorado network because we want to work smarter and not harder. We’re excited to streamline our referrals, as well as grow our own network of resources!
This is a momentous opportunity for our community and for the clients we serve. The Health District of Northern Larimer County is excited to support the Unite Colorado network, and to work with other organizations in the community to help realize the potential benefits of this network. We and many others have long dreamed about a better way to communicate about and coordinate referrals across organizations and systems. We look forward to deepening our community’s collaborative spirit, and enhancing access to the vast array of services available in Larimer County with this innovative tool.
At Kaiser Permanente we know that basic needs – like having a roof over your head and food on your table – dramatically affect a person’s ability to be healthy. As a health system we’re committed to improving the conditions for health and equity in the community by addressing the root causes of health, such as economic opportunity, affordable housing, safe and supportive schools, and healthy environments. We’re proud to work with Unite Us in providing a much-needed community network of resources to meet the critical needs of Coloradans.
Summit County is excited to partner with Unite Us, to improve access and streamline referrals among Summit County service organizations. The COVID-19 pandemic has hit our community hard and laid bare the need for deeper collaboration among partners. Often times people have multiple needs including housing, food access, and mental health. Unite Us will remove barriers, so that people receive needed supports faster. With a seamless referral system, community members will not need to tell their story over and over again, and the Summit Community can provide the wrap-around support needed in order to help our community heal.
Being a part of Unite Colorado tremendously helps connect the individuals we serve to the resources they need in a timely and trustworthy manner. The ease to navigate and find specifically what our members need really creates the peace of mind avoiding the ever so common “wild goose chase”!
Unite Colorado is the ideal platform that allows my organization to support youth and their families with their various needs! It is amazing that I can use Unite Colorado to research resources in our region that are up to date and connect community members.
I believe in partnering with organizations that lift up the work that is already happening in communities and strengthens grass-roots to grass-tops support. Unite Colorado fills those gaps by helping us identify solutions and track the success of our clients in getting the help they need.
United Way of Pueblo County has a longstanding history of serving as a connector for both nonprofits and individuals in our community. We are very excited to partner with Unite Colorado to better connect those in need with the resources available, while also making the referral process more streamlined for our hardworking nonprofit partners. Having a shared system will improve individual outcomes and provide critical localized data about needs which will help inform how we prioritize and leverage valuable community resources
In a time that has revealed inequity in access to services, Unite Us offers a vital tool for creating an equitable community ecosystem that offers all people access to services and knowledge in community resources. Wrap-around services are the way healthcare is heading and the way that community care should be. Unite Us offers a platform that our team can document in, case manage, and send referrals from. The client timeline has helped our organization stay up to date on where each client is at in their journey, and the ability to track referrals helps us to support them every step of the way.
