About Us

Unite Colorado is a coordinated care network of health and social service providers to address people’s social needs, and improve health across communities. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs and improve health across communities. 

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

The Network Continues to Grow Each Day

Join us as we onboard nonprofit, social service, and healthcare partners as the network continues to meet the needs of the community.

Summit County is excited to partner with Unite Us, to improve access and streamline referrals among Summit County service organizations. The COVID-19 pandemic has hit our community hard and laid bare the need for deeper collaboration among partners. Often times people have multiple needs including housing, food access, and mental health. Unite Us will remove barriers, so that people receive needed supports faster. With a seamless referral system, community members will not need to tell their story over and over again, and the Summit Community can provide the wrap-around support needed in order to help our community heal.
