Unite Florida is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.
The network is supported by a Florida-based Unite Us team focused on community engagement, network health and optimization, and customer success.
Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite Florida is sponsored by the Health Care District of Palm Beach County, UnitedHealthcare Community Plan, and the Charles and Margery Barancik Foundation implemented with Sarasota Memorial Health Care System.

Even for the best-supported families, it can “take a village.” Unite Us is one of the key solutions for the First 1,000 Days initiative, to ensure families have the support they need to help their children thrive. It is not only an outcomes-focused, closed loop electronic referral platform, but provides an accurate view of resources and a real-time data dashboard to view gaps in services and where to focus our initiative efforts. The launch of Unite Us in Sarasota County was extraordinarily successful, which speaks to the power of partnerships in our community. I am honored to be part of such a large, countywide system change that will have an impact for many years to come.
Unite Us has made it a lot easier and more streamlined to get folks to needed services. At the Healthcare District of Palm Beach County, we don’t have traditional case managers, but our nurses sometimes can help. Now, the platform has helped act as our AI type of case manager. I no longer have to struggle and lock myself into a phone call trying to get a patient to a referral source.
We are able to help more people through the process. We have quicker access to them and the ability to identify if we can serve them or if we just need to refer them. There is no more wait time for the client or for us, we are connecting them to care more efficiently. Unite Us has reduced those wasted hours for us, keeping everything we need in one place and allowing us to expand.
Since Unite Us, our capabilities to help people in a quick manner has expanded. We try to help as much as we can, so we are grateful that this particular program exists. Now we will be offering better, more, and quicker services to people because we have the power of Unite Us.
At 2-1-1 Broward, addressing the Social Determinants of Health and identifying a technology solution to streamline coordinated care delivery have been top priorities. With the understanding that effectively reducing health disparities improves population health outcomes, achieving racial health equity is stated in our C3 initiative’s goals and visioning. Building a robust network is the essential next step in co-creating systems change.
