Building connections for a healthier Massachusetts.

About Us

Unite Massachusetts is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

The network is supported by a Massachusetts-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Local Sponsors

Unite Massachusetts is sponsored by Merrimack Valley Accountable Care Organization My Care Family. My Care Family is a Massachusetts Medicaid Accountable Care Organization comprised of partner organizations Greater Lawrence Family Health Center, Lawrence General Hospital, and AllWays Health Partners.

What People Are Saying

As an app-based online drug & alcohol treatment program, a large part of our offering includes linkage to care. Having one tool to connect our members with local community partners is a game-changer. All of us at Halcyon Health are super passionate about supporting our members locally as it is a large part of their ongoing recovery journey. For us, joining Unite Massachusetts was about connecting with partners and taking the extra step to support our members’ journeys in a more efficient and effective way.

Director of Growth, Halcyon Health

When joining Unite Massachusetts, Uncommon Threads was looking to open up new connections, learn about programs that can benefit our clients and community, and support Unite Us’ mission of providing better coordination of social and health care services. We were drawn to the network as a means to expand our referral base, increase the number of clients we serve, obtain tools for measuring impact, and have one place to refer our clients to needed services.

Founder and Executive Director, Uncommon Threads

At Vincere Health, we help patients who are struggling with smoking/vaping addiction and need accessible support delivered remotely. We are always looking to get the word out and help more clients, and Unite Us makes it easy for organizations to simply refer their clients to us and vice versa. We’re excited for more organizations to join Unite Us and increase referral flows, as there are so many wonderful providers and community partners in Massachusetts who would benefit from having a more seamless way of pointing patients to all of the right services they need.

CEO, Vincere Health

In 2017, Brighton Marine answered the call to find a way to simplify how veterans, military members and their families find community support. Partnering with Unite Us and several community partners, we launched Coordinated Veterans Services to remove the burden and frustration from those searching for assistance. We are excited to see that we continue to break down barriers and strengthen collaboration with every new organization that joins this effort.
