Building connections for a healthier Oklahoma.

About Us

Unite Oklahoma is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

The network is supported by a Oklahoma-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Local Sponsors

Unite Oklahoma is sponsored by the Oklahoma Health Care Authority, Mercy Health, and CREOKS Health Services.

The Network Continues to Grow Each Day

Join us as we onboard nonprofit, social services, and healthcare partners as the network continues to meet the needs of the greater community.

What People Are Saying

Thanks to the Unite Oklahoma network, I never have to tell a veteran ‘no.’

Erica Walker
Executive Director at Warriors For Freedom Foundation

ReMerge is excited to use the Unite Us Platform to improve referrals between community partners. We believe that improving our coordination and information sharing will lead to greater efficiency and, most importantly, better outcomes for the clients our community collectively serves.

Jenna Morey
Executive Director, ReMerge Oklahoma

Funding from the CARES Act enabled Positive Tomorrows to implement the Unite Us Platform and band together with our partners to better meet the needs in our community. The platform allows service providers to collaborate in a more meaningful and lasting manner. Rather than simply pointing a client to another agency, we can follow their progress and work cooperatively toward a shared set of goals.

Kelly Berger
Director of Family Support, Positive Tomorrows

OIC has always understood the importance of providing wrap-around services to our clients in conjunction with the education and training we provide. As a Unite Us partner, we are able to focus on what we do best and still meet the needs of our students. In helping someone learn computers or get their high school equivalency diploma, Unite Us and the other partners help us to focus on that mission while allowing other partners to focus on theirs! Unite Us is good for Oklahoma City, and we are glad to be a part of it!

DesJean Jones
Executive Director, OIC of Oklahoma County
