The New Hampshire Care Connections network and Unite Us platform is available to health care and human services providers and community-based organizations across the state. Get in touch here to learn more.

No! Through the New Hampshire Department of Health and Human Services’ investment in Unite New Hampshire, eligible healthcare providers, health systems, and community-based organizations across the state can join and participate in the network at no cost. Integrations with EHR systems such as Epic, MEDITECH, and athenahealth, as well as Single Sign-On (SSO), are available and should be evaluated through both workflow discussions and technical feasibility assessments.

Healthcare providers, health systems, community-based organizations (CBOs), government agencies, and other organizations that support the health and well-being of New Hampshire residents are welcome to join the Unite New Hampshire network.
Organizations must agree to participate in the network according to established privacy, security, and data-sharing standards via a Network Participation Agreement. Additional onboarding steps will be reviewed with your organization during the enrollment process.

Onboarding timelines vary based on your organization’s size, workflows, and technical needs. Many community-based organizations can complete onboarding and begin using the platform in less than a week. For larger healthcare organizations and health systems, timelines may vary depending on factors such as team size, workflow design, and technology considerations. During onboarding, the Unite Us team will work closely with your organization to understand your referral processes, operational needs, and existing technology systems.
Whether you plan to use the Unite Us platform as a standalone solution or explore integration opportunities, our team will help identify the approach that best supports your goals and ensure you are set up for success.

Participating organizations can securely send and receive referrals, coordinate services with network partners, and track referral outcomes through closed-loop referral workflows. Organizations also benefit from access to a statewide resource directory, self-referral tools, and reporting capabilities that help identify community needs, measure impact, and strengthen service delivery across New Hampshire.
