Building connections for a healthier Washington.

About Us

Unite Washington is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us’ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

The network is supported by a Washington-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Local Sponsors

Unite Washington is sponsored by the Community Health Plan of Washington, Kaiser PermanenteMultiCare Health SystemOptumCare, ESD 105, and WestCare.

WestCare also oversees the WAServes, a Unite Us-powered network that focuses on service members, veterans, and their families.

The Network Continues to Grow Each Day

Join us as we onboard nonprofit, social service, and healthcare partners as the network continues to meet the needs of the community.

What People Are Saying

When I started working here, it was overwhelming to see all the possible resources and try to decipher which is the best fit. That’s why I like Unite Us, because I can filter the broad list of resources to find the resources that are the best fit for the individuals that I’m working with.

Food Bank Manager, FamilyWorks Seattle

Uplifting the human spirit is a lofty statement for any organization to promote as their objective, especially given the challenging world we live in today. Quality health and human services for individuals, families and communities in need are a priceless resource. We honored to have a network of community providers who are dedicated to serving our active duty service members, veterans and their families. We are better together.

The fragmented landscape of health and social services often means that people aren’t receiving the total care they need to stay healthy, safe, and well. By building a public health infrastructure to meet people’s basic needs, Unite Us allows for collaboration, care coordination, and the ability to track member outcomes. We want to be able to ensure quality care for our members through their entire health journey, and the partnership with Unites Us will help all of a person’s care team stay on the same page.

Jennifer Polello
Senior Director of Quality and Population Health, Community Health Plan of Washington

Social factors like access to healthy food and safe, affordable housing have a major impact on health. Even before the pandemic, many people’s basic needs were not being met, and COVID-19 has only made that worse, particularly in communities of color. It is exciting to be part of a robust online platform, where referrals and ongoing coordination enable health systems and safety net resources to stay connected as we work together to take care to the next level for the people and families we serve.

Medical Director of Medicare and Medicaid, Kaiser Permanente Washington

The Unite Us partnership with ESD 105 will help our districts and schools track outcomes, and prevent students from falling through the gaps. We believe this element of seamless communication and outcomes tracking is essential to understanding referral effectiveness, and how well our students’ needs are being met. Our partnership with Unite Us will help our students and their families address social risk factors, and overcome systemic barriers to these critical resources.

Emily Contreras
Behavioral Health Coordinator, Educational Service District (ESD) 105

It is critical that we get clients set up on the right foot, but we were struggling to connect them to outside services without a gap in care. When we connected to the Unite Us Platform, it alleviated any confusion and helped us better support the goals of our clients. The platform minimizes the heavy lifting by holding both the referring agency and the organization accepting the referral accountable to follow-up with the client. This is what community-based organizations need to bridge the gap!
