Collaborating for a healthier Washington.

When I started working here, it was overwhelming to see all the possible resources and try to decipher which is the best fit. That’s why I like Unite Us, because I can filter the broad list of resources to find the resources that are the best fit for the individuals that I’m working with.

The fragmented landscape of health and social services often means that people aren’t receiving the total care they need to stay healthy, safe, and well. By building a public health infrastructure to meet people’s basic needs, Unite Us allows for collaboration, care coordination, and the ability to track member outcomes. We want to be able to ensure quality care for our members through their entire health journey, and the partnership with Unites Us will help all of a person’s care team stay on the same page.

Social factors like access to healthy food and safe, affordable housing have a major impact on health. Even before the pandemic, many people’s basic needs were not being met, and COVID-19 has only made that worse, particularly in communities of color. It is exciting to be part of a robust online platform, where referrals and ongoing coordination enable health systems and safety net resources to stay connected as we work together to take care to the next level for the people and families we serve.

It is critical that we get clients set up on the right foot, but we were struggling to connect them to outside services without a gap in care. When we connected to the Unite Us Platform, it alleviated any confusion and helped us better support the goals of our clients. The platform minimizes the heavy lifting by holding both the referring agency and the organization accepting the referral accountable to follow-up with the client. This is what community-based organizations need to bridge the gap!
