Unite Us Announces Inaugural Social Determinants of Health Summit

Introducing One Continuum: A Call to Action for Leaders Across the Health and Human Services Industry

New York, NY – Unite Us, the leading care coordination platform bringing together healthcare and social services, is hosting their inaugural annual Summit, “One Continuum,” on February 21, 2019. The invitation-only event will bring together senior leaders across healthcare, the community, government, and philanthropy to have a candid conversation about cross-sector coordination, social determinants of health innovation, and partnering in one ecosystem.

Attendees will represent various stages along the spectrum of implementing a social determinants strategy – whether that’s in early-phase, mid-implementation, or post-launch – and will also represent local, statewide, and national organizations across sectors and silos. Panelists were selected from organizations that have demonstrated groundbreaking programs, partnerships, and innovative models that are addressing the social determinants of health and delivering person-centered services at scale.

“The U.S. healthcare system is at an inflection point when it comes to transforming the way we deliver care. The narrative around achieving health has expanded beyond just healthcare to include social care. In order for this to work, we need everyone to have a seat at the table, and that’s what One Continuum is all about,” says Taylor Justice, President of Unite Us.
