RICHMOND, VA– Virginia Hospital & Healthcare Association (VHHA) affiliate VHHA Solutions> has entered into a new strategic partnership with Unite Us, the nation’s trusted technology partner for integrating health and community-based care. This new collaboration will support the growth of Virginia’s connected health ecosystem by facilitating seamless referrals and coordinated care across clinical providers and social service organizations.
Unite Us provides enterprise technology solutions to efficiently integrate the delivery of services across health care, government, nonprofit, and other sectors. Network partners use the Unite Us platform to securely identify, deliver, and pay for services that address the needs of individuals within their communities. Through Unite Us, hospitals and health systems can build and scale coordinated care networks, track outcomes, identify service gaps and at-risk populations, and empower members of the community to take ownership of their own health.
VHHA Solutions will work with Unite Us to better connect Virginians to vital community resources that support whole-person health, such as housing, food, employment services, behavioral health, and transportation. Unite Us ensures individuals receive timely, accountable, and compassionate care while reducing gaps in service delivery.
In one successful program implementation, Ballad Health, in collaboration with VHHA and the Virginia Department of Health, deployed Unite Us’ closed-loop referral system to support patients with health-related social needs. A recent analysis of the results of that work found a 16.2 percent average decrease in emergency department (ED) visits among all tracked patients in the six months following a social care referral. The study also found that average ED visits by patients who received a social care referral and were engaged by a Ballad community health worker (CHW) decreased 24.8 percent in the following six months. These outcomes underscore the importance of referral technology, seamless data integration, and coordinated care delivery.
