About Us

NH Care Connections (NHCC) is a network of health care and human services providers and community-based organizations supporting individuals’ and families’ needs in real-time by:

  • Reducing the time and effort to locate and refer people seeking care to the services they need.
  • Protecting each person’s right to confidentiality and to consent to share personal information.
  • Ensuring people receive effective follow-up to ensure the best health outcomes.

The goals of the network include improving people’s experiences navigating the healthcare system and improving health outcomes.  Creating more pathways for communication and stronger partnerships is essential to ensuring the best health outcomes for people and their families.  Streamlining the process of referring people for care at healthcare facilities and community-based organizations is essential to improving people’s experiences in the healthcare system.

The Network Continues to Grow Each Day

NH Care Connections was established by the NH Department of Health and Human Services and the NH Department of Military Affairs and Veterans Services.  The network includes providers that offer services such as healthcare, housing, employment, food assistance, mental health, utilities, education, veteran services, and more.

What People Are Saying

We often only have a person in front of us for a few minutes at a time, so to be able to do everything we can to help them in that window is really important. Previous to Unite Us, in order for us to help a participant in all the ways we wanted to, we had to make eight or nine different referrals, get different releases, and/or make different phone calls. But with the security of Unite Us, we know that relevant information has been uploaded and consent has been received. We can work within a system with everything in one place at one time. In a five-minute window, we can help a person do eight different things that would have otherwise taken us a few days.

In order for a client or in my case a resident to get the highest level of care necessary, there needs to be solid communication and inter-connectedness within all of the resources.
