About Us

In New Hampshire, we’re building a statewide coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

We often only have a person in front of us for a few minutes at a time, so to be able to do everything we can to help them in that window is really important. Previous to Unite Us, in order for us to help a participant in all the ways we wanted to, we had to make eight or nine different referrals, get different releases, and/or make different phone calls. But with the security of Unite Us, we know that relevant information has been uploaded and consent has been received. We can work within a system with everything in one place at one time. In a five-minute window, we can help a person do eight different things that would have otherwise taken us a few days.
