Building connections for a healthier Texas.

About Us

Unite Texas is a coordinated care network of health and social service providers. Partners in the network are connected through Unite Us‘ shared technology platform, which enables them to send and receive electronic referrals, address people’s social needs, and improve health across communities.

The network is supported by a Texas-based Unite Us team focused on community engagement, network health and optimization, and customer success.

Joining the network is at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.

Local Sponsors

Unite Texas is built in partnership with Alamo Area Council of Governments (AACOG), the University of Houston College of Medicine, Texas Association of Community Health Centers, and Titus Regional Medical Center.

AACOG also oversees the Texas Veterans Network, a Unite Us-powered network that focuses on service members, veterans, and their families.

What People Are Saying

All of the success Texas Veterans Network has had would not be possible without the support of our valued partner, Unite Us. I want to express my gratitude to Unite Us for their leadership and outstanding service to the 27 counties that the Alamo Area Council of Governments (AACOG) provides through this partnership. We look forward to working together alongside community organizations in serving and connecting our nation’s best to the resources they need.

The mission of the University of Houston College of Medicine is to improve the health and health care of the population of the city of Houston and the state of Texas. By working with the Unite Us platform, we will be able to effectively and efficiently connect those with needs to our participating network organizations who can help to address those needs. This helps us to achieve our mission, and is the reason that we chose Unite Us as our partner in this effort.
