About the Summit

Industry pioneers. Community champions. Tech trailblazers. One Continuum is the only summit dedicated to uniting stakeholders influencing the care continuum. For one day, leaders from healthcare, health plans, government agencies, and social service organizations come together to exchange and inspire ideas that drive meaningful change.

The 2024 program is meticulously curated to address the unique challenges and opportunities shaping our field, spanning regulatory shifts, program advancements, emerging technologies, and more. Join discussions guided by industry leaders and peers; gain insights from the community organizations on the front line; and explore Unite Us’ innovations.

One Continuum is centered on our vision to improve people’s health and well-being by connecting communities through technology. We believe that when we work together, anything is possible. Are you in?
