Georgia’s Closed-Loop
Coordinated Care Network

Together, we’re connecting care across Georgia—linking organizations, sharing resources, and ensuring every individual and family has access to the support they need to thrive.

About the Network

Where Care and
Community Come Together

Unite Georgia unites organizations across the state to meet the needs of individuals and families with compassion and coordination. Together, we’re building stronger connections, improving access to care, and empowering communities to thrive.

  • Coordinate Care Seamlessly
Connect people to verified services and close the loop on every referral, documenting client outcomes not process-related outputs.
  • Collaborate Across Sectors
Break down organizational silos, working together with healthcare, government, and community partners.
  • Show your Impact
Access data and insights that help you advocate for your community, turning referral and case data into a strategic asset for planning, budgeting, and legislative engagement
How the Network Works

Connection, Clarity, and Capacity.
All in One Network.

Stop Guessing.
Guarantee Care with a Unified Network.

Too often, referrals disappear into the void—leaving organizations to work off assumptions, unsure if someone received the help they needed. The Unite Us platform changes that. With secure, closed-loop referrals, your team can connect individuals to verified services, track progress in real time, and be confident that client needs are met.

Fragmented Systems Fail. Get the Full Picture to Meet 100% of Community Need.

Unite Us Insights leverages unified network data to provide a clear, no-cost picture of service utilization and gaps, allowing your organization to advocate for systemic change based on evidence.

Stop Wasting Time.
Scale Your Mission with a Single, Integrated Solution.

Organizations can choose to purchase advanced tools like Integrations, and analytics features to seamlessly embed social care into existing systems and deepen the capacity of their team.
