Unite New York and Unite NYC are a coordinated care network consisting of healthcare, government, nonprofit, and other organizations. These network partners use Unite Us’ software to securely identify, deliver, and pay for services that address the needs of individuals within their communities.
The network is supported by Unite Us team members focused on customer and community engagement, network health and optimization, and user support.
Joining the networks are at no cost for community-based organizations and many organizations that are considered part of the safety net, like community health centers, tribal clinics, and mental health centers.
Unite New York and Unite NYC are sponsored by Adirondack Health Institute, Healthy Alliance, Catholic Charities Family and Community Services, the NYC Department of Veterans’ Services, Public Health Solutions, Mayor’s Office of Criminal Justice, Sun River Health, AHN RHS, EngageWell IPA, Coordinated Behavioral Care, Guthrie Clinic, Sullivan County, NYC Department of Health & Mental Hygiene, and Maimonides Health.

The New York City Department of Veterans’ Services also oversees VetConnect NYC, a Unite Us-powered network that helps service members, veterans, survivors, and families find the resources they need to live fulfilling and purpose-driven lives.

For over 100 years, Catholic Family Center has strived to fulfill its mission to support the poor and most vulnerable achieve their fullest human potential. We know this goal can’t be attained through the work of any one service or safety net provider, so we are committed to driving the expansion of Unite Us network of providers so that the most vulnerable in our community can have access to what they need to thrive.
It would be impossible to have the connections to as many services as the network can link us up with. It goes back to the time consumption. Even if my “network” was as large as the Unite Us database, I would not have enough time in the week to reach out to them. The referral is sent from organization to organization seamlessly. It has been very efficient for myself and the families we serve.
We are excited to partner with Unite Us and look forward to supporting the company’s continued expansion throughout the Western New York community. We believe the company’s commitment will drive accelerated growth and provide access and tools that are normally not available.
Social needs have a direct and tangible impact on health. By mobilizing networks across one platform, we are able to connect people to SDoH services. Through the great work being done across social care providers to improve the health and well-being of individuals and communities, we will see improved health and reduced costs for our state. Together, we will continue to break down barriers between medical and social care providers.
What started out as a solution for an emerging COVID-19 related emergency food need morphed into an internal and exterior referral strategy for multiple services. We chose Unite Us because of their support and flexibility when faced with our changing needs. Unite Us works hard to make us feel connected with the rest of our network and tailor their service to our needs. They do not provide a ‘cookie-cutter’ service. They are quick with their correspondence and have consistently risen to provide additional trainings for our staff, allowing us to make the most of their platform. They are a true partner that we are proud to work with.
PHS, our healthcare partners, and our diverse community-based organization partners have built the most active, technology-enabled, community resource network in NYC, connecting hospital patients and managed care members to local resources that support them in resolving their unmet, health-related social needs. Unite Us allows us to track assessments and outcomes across organizations and sectors. During the pandemic, the network connected more than 10,000 people discharged from the hospital with COVID-19 or other chronic conditions to resources to address their non-medical needs.
Over the years of working with Unite Us, we have been fortunate to see how connecting with other NYC providers improves outcomes for our clients. The Unite Us Platform also gives us the opportunity to serve more clients that we may have never have met otherwise.
