DSRIP PPS Alliance For Better Health Partners With Unite Us to Improve Health Outcomes in Upstate New York

National Press Releases

The coordinated care network will connect Alliance for Better Health’s clinical and social service partners together to address patients’ social determinants of health and reduce avoidable hospital admissions.

NEW YORK — Unite Us, a Veteran-owned technology company, announced that it has partnered with Alliance for Better Health (Alliance) as the software provider that will power Alliance’s coordinated community-wide care network.

Alliance’s partners will be delivering care as part of an integrated and accountable network of both healthcare and social service providers. Providers will be able to electronically connect patients to community partners, and report on every outcome achieved inside and outside the clinical setting.  This empowers Alliance and its partners to directly address patients’ social determinants of health, ensuring patients are at the center of care, while tracking their health journey across the community.

“This partnership with Unite Us will continue to fuel the needed transformation that has begun in NY state and across the country,” says Colleen Russell, Chief Health Information Officer/CIO at Alliance. “By increasing collaboration, accountability, and efficiency among our community providers as well as gaining the much-needed visibility and tracking of the continuum of care and social determinants of health for our patients, we are poised to see a reduction in avoidable utilizations, readmissions, and overall improved outcomes.”

The Unite Us platform will allow Alliance’s clinical partners throughout Albany, Fulton, Montgomery, Rensselaer, Saratoga, and Schenectady counties to connect in real time with social service providers such as housing and workforce development agencies, behavioral health providers, and food pantries.

All partners across the network will be centrally connected to each other, with the ability to:

  • Electronically connect patients to both clinical and social service providers.
  • Track patient progress & receive automated feedback from partners to ensure care and services are received.
  • Collaborate with community-wide “care teams” ensuring providers are on the same page regarding their shared patients’ progress.
  • Collect data including structured patient outcomes to measure the network’s impact, including time to service and effectiveness of care.

“Alliance knew they needed to go beyond screening and identifying social needs in order to track and improve outcomes,” says Dan Brillman, CEO of Unite Us. “This partnership solves for the connectivity the community has needed and offers a long-term solution by implementing a framework that views clinical and social services as part of one singular continuum.”

Network development is already underway, and Unite Us is currently working closely with both Alliance and its partners to optimize workflows for each partner, standardize community-wide data and reporting, and train all participants on how to use the platform effectively. Alliance’s clinical partners will now be fully connected with their community partners, including the United Way of the Capital Region/211 and Catholic Charities of the Diocese of Albany, among many others.
